Confidential Practice Information: All practice addresses, physician workforce data, and financial details submitted to CELSIANT Health Advisory via the Eligibility Assessment form are treated as strictly confidential client information and are used solely to evaluate federal HPSA, NHSC, State SLRP, and Conrad 30 program eligibility.
Advisory Scope Notice: CELSIANT Health Advisory provides healthcare workforce strategy and funding navigation consulting. Final grant, loan repayment, and visa waiver approvals are determined by the respective federal and state agencies (HRSA/NHSC, Well-Ahead Louisiana, and state health departments).
